Online Entry Form
Artists around the world are encouraged to use the online entry form. The online entry form is available in English, Spanish, German, French, Japanese, Russian, Korean, Portuguese, Italian, and Chinese. Artists will specify a language choice during the online sign-in process. An LDS Account is required in order to access the online entry form. If you do not have an LDS Account, go to Online Submission Instructions to learn how to create an account.

Printed Entry Form
Entry forms are also available to print as PDF documents in Chinese, English, French, German, Japanese, Korean, Italian, Portuguese, Russian, and Spanish. Printed entry forms and photographic images can be sent by regular mail and must be postmarked on or before October 10, 2008. Do not send original art.
