My signature does not match the name printed on my passport. Is that a problem?

It will not be a problem.

Can I make corrections to my visa forms if the information already filled in is not correct?

You may use white out tape (not liquid) to make needed changes. Do not scribble or cross things out.

How do I attach my photo to the application?

Use a glue stick to attach your photo. Tape or staples are not acceptable.

How do I complete my visa application?

Complete as noted on the instruction sheets.

What does the medical letter from the doctor's office need to say?

The letter must simply state the following—nothing more, nothing less:

“I, [doctor’s name] have examined [missionary’s name] and found [him/her] to be healthy and free of contagious diseases.”

The letter must be signed and dated by the doctor on the doctor's office letterhead.

Also, missionaries should make a photocopy of the completed immunization form found in their original call packet.

Why does the mission office need my birth certificate?

The birth certificate is kept by the mission office and will be given back to missionaries when they return home. The mission office prefers to have one that has been issued within five years of a missionary's mission call, but it is not mandatory for it to be so.

If missionaries have some of their visa documents completed, can they mail some now and mail the rest later?

No, missionaries should wait until they can send all of the documents at once.

If missionaries didn’t turn their visa documents in on time, will they be delayed?

Missionary Travel must receive all documents at least 30 days before missionaries enter the missionary training center. Otherwise, missionaries may be delayed.

Can missionaries keep their passport for a personal vacation?

Yes. Missionaries do not need to send in their passport beforehand. They should bring their passport with them when they report to the missionary training center.
