My signature does not match the name printed on my passport, is that a problem? 

No. If you did not sign your full name on the passport, all your visa documents must be signed the same way you signed your passport.

Do missionaries need to send in their actual passport?

Yes. Your actual passport is needed in order for Missionary Travel to apply for your visa.

Can I make corrections to my visa forms if the information already filled in is not correct?

You may use white-out tape (not liquid) to make needed changes. Do not scribble or cross things out.

How do missionaries complete the parts of the visa application that there are no instructions for?

Missionaries MUST NOT complete anything on their visa application that they are not specifically instructed to complete.

Section VI asks about family. If missionaries have more siblings than there are spaces, what should they do?

Missionaries may attach another sheet of paper with the rest of their siblings’ information on it. Missionaries should include two copies of this sheet, one for each application.

Section IX talks about education. Should missionaries really put their high school information? What if missionaries have gone to a little bit of college, but have not completed their degree?

Missionaries should fill out the information for the highest level of education that they have completed. In other words, they should fill in the information for the highest degree that they have already attained.
