No, a visa is not required for Puerto Rico.

A visa is not required for missionaries' training at the Dominican Republic Missionary Training Center. Their passport will be sufficient to enter the Dominican Republic.

If missionaries are going to the Dominican Republic Missionary Training Center, do they send in the flight request card?

No, that card is only for the Provo Missionary Training Center. Missionary travel will automatically book a flight for missionaries and either mail or email the itinerary to missionaries. Missionaries will receive a final printed itinerary with their passport and visa three to seven days prior to their travel.
